5D-Treatment of Class II Malocclusion

This presentation explores patient‑related determinants that may enhance the efficacy and efficiency of Class II treatment in growing patients. Specifically, it focuses on two key dimensions: (1) treatment timing, conceptualized as the fourth dimension in diagnosis, and (2) individual patient responsiveness, conceptualized as the fifth dimension in diagnosis.
Dr. Franchi will demonstrate that functional jaw orthopedics (FJO) for Class II malocclusion can effectively influence both short‑ and long‑term mandibular growth and the sagittal position of the mandible, when active treatment is carried out during the pubertal growth spurt. To anticipate individual responsiveness to FJO, mandibular morphology at puberty should be carefully assessed. Ideal candidates for functional jaw orthopedics in Class II cases with mandibular retrusion typically exhibit a small mandibular angle (Co–Go–Me), which is associated with favorable mandibular growth potential.

Prof. Dr. med. dent. Lorenzo Franchi
Full Professor of Orthodontics and Dean of the School of Dentistry at the University of Florence. One of the leading experts on growth modification and treatment timing in Class II malocclusion.
Dr. Lorenzo Franchi completed his dental education and his PhD program in Preventive Orthodontics at the University of Florence in 1989 and 1997, respectively. In 2008, he completed his specialization in Orthodontics at the University of Rome “La Sapienza”. Dr. Franchi is currently Full Professor of Orthodontics, Dean of the School of Dentistry, and Director of the Postgraduate Orthodontic Program at the University of Florence, Italy. He also serves as a Thomas M. Graber Visiting Scholar at the Department of Orthodontics and Pediatric Dentistry, University of Michigan, Ann Arbor (USA), where he has collaborated with Dr. James A. McNamara since 1995.
He has lectured extensively at international congresses and symposia on orthodontics and dentofacial orthopedics and has published more than 300 articles in international orthodontic journals. Dr. Franchi served as Editor of Progress in Orthodontics from 2011 to 2012 and is currently Associate Editor of both the European Journal of Orthodontics and Progress in Orthodontics.
He is a Past President of the Italian Society of Orthodontics (SIDO). His main research interests and areas of expertise include early treatment, treatment timing, and dentofacial orthopedics.
