Surgery First in Orthognathic Treatment

The Surgery‑First Approach (SFA) has emerged as an alternative to conventional orthognathic treatment, offering shorter overall treatment time and immediate facial improvement by performing orthognathic surgery before any pre‑surgical orthodontic decompensation.
This lecture aims to provide key perspectives on SFA for the orthodontic team. Clinical and research outcomes from current literature, recent systematic reviews and meta‑analyses, as well as proposed clinical protocols will be presented. Several topics will be addressed and discussed: What is the biological rationale behind SFA? Who is – and who is not – a candidate? What changes in our workflow? Is post‑surgical stability truly comparable to the conventional approach?

Harald Essig (*13 June 1975) is a German‑Swiss specialist in oral and maxillofacial surgery. After studying human medicine in Greifswald and Munich as well as dentistry in Kiel, his clinical career led him via Kiel and Hannover Medical School to the University Hospital Zurich in 2014.
Since then, he has first served as senior attending physician and vice director of the Department of Cranio‑Maxillofacial and Oral Surgery before becoming acting director in 2020; in 2026 he was appointed full clinic director. He obtained his habilitation at Hannover Medical School in 2014 and was granted the venia legendi at the University of Zurich in 2016. In 2021, he was appointed titular professor, and as of 1 March 2026 he holds a professorship in oral and maxillofacial surgery at the University of Zurich, combined with the leadership of the clinic at University Hospital Zurich.
